 Don't underestimate any missing tooth

A missing tooth, causing a gap between the teeth, affects not only the aesthetics of your teeth but mainly its functionality. Unwanted changes are taking place in the dental arch as well as in the entire oral cavity:

  • Teeth adjacent to the space begin to move towards the gap and group together over time
  • Opposite teeth tend to grow due to insufficient back pressure
  • Bone withdrawal (atrophy occurs)
  • Impaired articulation
  • Worsening of food intake
  • Jaw deformation or joint problems due to uneven load of teeth (if teeth are missing in the back of the arch)

 

When you loose a tooth, you loose also the root of your tooth. The implant fully replaces the missing tooth. The root replacement part looks like a tiny screw that can be used both for the upper and lower jaws.

It is made of titanium or other materials suitable for use in the human body. The implant is fixed inside the bone to provide a stable base for the new tooth - dental crown, or even for multiple teeth replacement - dental bridge.

Unlike removable dental replacements, implants are permanently integrated within the bone and do not need to be removed for the night. The advantage is that, if necessary, the crown or bridge can be removed easily and pain free, be repaired in our laboratory and put straigt back.

What does the new tooth consist of

It is the implant itself, which is inserted directly into the jaw bone, and thus is not
visible.

This part is visible during the healing phase. It is a small screw that covers the entrance to the inner thread of the implant. After the healing and preparation of the final tooth (i.e. crown) is over, the screw is removed for a tailor-made crown.

The dental crown is a visible part of the tooth - it's a new fully functional ceramic tooth. It is both a functional and aesthetic substitute for the missing tooth. The implant is indistinguishable from your own tooth.

The implant material is crucial

The material the implant is made of is the alpha and omega affecting not only the length of the healing time, but also the overall success of the procedure. At Schill Dental Clinic, we work with titanium implants of the renowned Swiss brand Straumann with active biosurfaces. This reduces the healing time to half with 99 % success rate. With cheaper implants with different surface treatment, the percentage of success is lower and the healing time longer.

Steps to a new tooth

A comprehensive dental examination is required before each procedure. At the Schill Dental Clinic, we will take X-ray and CT images, and immediately assess dental conditions and bone quality. The dental surgeon will explain the course of the procedure, duration and financial part, any possible risks and rules that need to be observed during the treatment in each phase.

Plague and tartar must be removed from the teeth. A specialist in dental hygiene painlessly, using ultrasound, completely removes all the dirt from the teeth, including dental plaque or pigmentation, to make the environment completely clean.

With painless surgery, lasting a few minutes, the dental surgeon inserts the implant into the jawbone with only a slight pressure. In this way, several implants can be applied at the same time. The procedure is performed under local anaesthesia.

After implantation, you experience mild swelling or not. If swelling occurs, it recedes 2-3 days after the procedure. There is also a mild sensitivity at the site of the intervention, which gradually ceases within about a week. At this time, it is recommended to eat mashed food, not to smoke or drink alcohol. At the front desk, you will receive all the instructions, the observance of which will facilitate your healing.

Approximately one week after the procedure, the second visit to the ambulance is followed by stitches removal - if needed during the intervention. Often, the implant is fixed so that no sewing is necessary and this phase is avoided.

Healing lasts until the third visit of your stomatologist. It usually takes around 6 weeks to heal completely. A missing tooth can be replaced with a temporary crown, dental bridge or removable dental replacement. The patient thus recovers with a complete smile.

The implant stability measuring device measures the degree of adhesion. If all is OK, the dental surgeon will take the imprints and make the final dental crown. It is made in our internal laboratory.

A few days after the impressions were taken, during the fourth doctor’s visit, the final crown is screwed into the already inserted implant (the new tooth root). If the crown is not perfect, it can be easily and painlessly unscrewed, the dental technician will adjust it and your doctor will put it back without any difficulties. The smile is beautiful and the dentition is fully functional.

Periodic check-ups are required after the dental crown has been placed on the implant. We focus on the condition, possible development of a dental bite, chewing and dental hygiene.

Lifespan of dental implants

With a proper care and excellent work, you can count on their lifetime use. Proper care and hygiene, as well as regular professional cleaning in the outpatient clinic are important once every six months. The fact that the implant is an inanimate part does not mean that it is not necessary to look after it. The implant requires the same level of hygiene as your own teeth.

In an exceptional case, it may happen that the implant "heels off", meaning it will fall out. This is usually the case at the beginning, during the healing phase after implantation, in about 1-1.5% of cases. Nothing is lost, the bone simply grows back. Consequently, in a relatively short time, the implant can be inserted again and successfully. In this case, you do not pay for the reinsertion.

What are your most frequent questions

Implants are intended for adult dentition, when growth and development is completed, that is to say from the age of 18 years. The suitability of the implant is assessed by a specialist - dental surgeon, based on a CT scan and 3D bone image and adjacent structures.

Implants are not suitable for diseases that worsen the conditions for their healing.

For example:

  • Patient has undergone a facial radiation
  • When suffering from severe diabetes
  • When suffering from periodontitis
  • Heavy smoker

However, it should be noted that if, for example, diabetes or osteoporosis is under control, properly treated and stabilized, it is possible to use dental implants with comparable success rates as achieved in healthy patients. The same applies to acute or long-term inflammation in the oral cavity. First you have to remove the problem and then decide what to do next. In any case, proper communication with the doctor is required.

Titanium is a pure metal, tastes neutrally (i.e. it does not have a "metallic" aftertaste). It does not cause any allergic reactions in the body and is therefore commonly used in medicine. The titanium implant is firm and resistant to pressure and stress generated by chewing. It is also not subject to corrosion. We recommend the Straumann implants, which are the leading innovations in surface treatments and treatment methods. These implants are verified not only clinically but mainly practically. Due to their unique properties, they are also used in high-risk patients.

Many patients are interested in how the human body reacts to a foreign object inserted in the bone or whether an allergic reaction can occur. The implants have different surface modifications of the parts inserted into the bone. We use titanium implants with an active biosurface for rapid healing. The implant needs some time to adapt. For healing, which lasts from six weeks to three months, the implant is osseintegrated and even partially gets into the bone structure. The crown, bridge, or prosthesis can be safely attached to this firmly held root.

Due to their material, the implants are durable and highly resistant, while retaining their original aesthetic effect. After surgery and recovery, the patient has no problem to eat and articulate normally. The implant can replace either one or several teeth. It can serve as an abutment for a dental bridge replacing several teeth next to each other.

If a patient decides for implants, they must consider surgery, even if painless, and a higher price for the implantation of multiple teeth.

Bridges have also several advantages. The procedure does not interfere with the bone and can be used, for example, by cancer patients if the possibility of implants has been excluded. The price for more tooth replacement may be more pleasant, but it does not have to pay off in the end. One of the disadvantages when using dental bridges is the necessity of preparing healthy support teeth. Dental replacement using a bridge will show a complete smile, but it does not guarantee its health. Since the bridge replaces only the tooth crown, not the root, problems may occur in the gums and jaw. If the bridge is damaged, it must be taken off and repaired as a whole.

If there is enough of the bone left, the implant can be introduced immediately. With a good bone quality, we will also make a temporary crown - so the patient will leave with a new tooth that does not look like a replacement, and it is unrecognizable from the final one. You can eat when having a temporary

replacement, however, you must remember it is just a temporary solution. The doctor will then change the provisional replacement for a permanent tooth. If the bone is missing to ossointegrate the implant - we can help also in this case. Just one visit and the dental surgeon can create suitable conditions for inserting the implant by adding the bone and in most cases also introducing the implant. If your doctor decides that you need to wait for a graft healing or augmentation, the implant will be fixed in the healed and completed bone at your next visit. The procedure is pain-free and performed under local anaesthesia. If the patient wishes or in case of intensive or extensive interventions, dealing with a reconstruction of the whole dentition, we will provide general anaesthesia.
